Gentiva Hospice provides end-of-life care for individuals with a terminal illness, focusing on comfort as well as pain and symptom management.
Our team of physicians, nurses, nurses aides, chaplains, volunteers and social services stand ready to customize a care plan for each person's unique situation.
To be eligible, a patient must have a terminal illness with a life expectancy of six months or less (as certified by their doctor) and elect hospice care, understanding it focuses on palliative care rather than curative treatment.
Routine Home Care:
This is the primary level of Gentiva hospice care, provided in the patient’s home.
Nursing Facility:
When a person calls an assisted living or skilled facility home, hospice can also be provided in addition to services from the facility.
Inpatient Respite Care:
Short-term care in a hospice inpatient facility to provide relief for family caregivers.
Gentiva also provides medications for pain and symptom management, medical supplies such as pull ups, and medical equipment such as a hospital bed.
