ACCESS Family Care is seeking a compassionate and skilled Behavioral Health Consultant (BHC) to join our integrated primary-care team. This role provides brief, solution-focused behavioral health interventions directly within the medical clinic setting, supporting patients alongside physicians, nurses, and care teams.
Position Overview
The Behavioral Health Consultant works as part of an interdisciplinary team to address the behavioral, emotional, and psychosocial needs of patients. This position focuses on brief interventions, warm handoffs, and collaborative care to improve whole-person health outcomes.
Key Responsibilities
Provide brief behavioral health interventions using evidence-based approaches such as CBT, MI, and solution-focused strategies
Conduct same-day warm handoffs from medical providers for immediate patient support
Perform behavioral health assessments including risk evaluations and diagnostic screenings
Collaborate closely with medical providers to develop integrated care plans
Document patient encounters efficiently within the electronic health record
Support patients with chronic disease management, lifestyle changes, and mental health needs
Participate in care team huddles, case reviews, and quality-improvement initiatives
Qualifications
Master’s degree in Social Work, Counseling, Psychology, or related field
Active Missouri licensure (LPC, LCSW, LMFT, or provisional equivalent)
Experience providing brief therapy, crisis intervention, or integrated care preferred
Strong communication skills and comfort working in a fast-paced clinical environment
Ability to collaborate effectively with medical and behavioral health teams
